HHT affects roughly one in 4,000 people, and about 80% remain undiagnosed. The pattern behind the Curaçao Criteria is recognizable in primary care, ENT, hematology, obstetrics, and pediatrics: spontaneous recurrent epistaxis, mucocutaneous telangiectases at characteristic sites, visceral AVMs or VMs, and a first-degree relative with HHT.

The clinical stakes sit in what is silent. Pulmonary AVMs are present in at least 40% of people with HHT and frequently produce no symptoms until a stroke or brain abscess. Screening finds them while they are still treatable.
